Known as SASH for ALL, this expanded model will work in partnership with adults and families living in affordable housing to increase resilience, strengthen wellbeing, improve health outcomes, and build community. We are looking for full-time, part time or contract registered nurses to work with a dedicated team including a community health worker and peer support advocate to:
o Pioneer a model to provide prevention-based health and wellness services and support to local housing communities;
o Help design and test the key elements and determine best practice;
o Innovate and collaborate with team members. community partners and participating adult and families to help the whole community thrive.
Successful candidate must be currently licensed as a Registered Nurse in the State of Vermont. Must possess at least two years of nursing experience [MD1] and be committed to a strength-based person-centered philosophy. Must be able to work independently, but thrive in a team-based environment and possess strong organizational and time management skills and excellent verbal and written communication skills. Wellness coaching, group learning facilitation and proficiency and comfort with computer software are important aspects of this position.
