We are introducing an expanded version known as SASH for ALL, which will focus on adults and families living in affordable housing. Our goal is to enhance resilience, strengthen well-being, improve health outcomes, provide health coaching, and foster community connections.
We are seeking a part-time or contract nurse for 20 hours per week to work alongside a dedicated team that includes a community health worker and a social emotional wellness clinician.
In this role, you will:
- Help pioneer a model that provides prevention-based health and wellness services and support to local housing communities.
- Assist in designing and testing key elements of the program to determine best practices.
- Collaborate with team members, community partners, and participating adults and families to promote community well-being.
Successful candidates must be licensed as a Registered Nurse in the state of Vermont and have at least two years of nursing experience. A commitment to a strength-based, person-centered approach is essential. You should be able to work independently while also thriving in a team environment. Strong organizational and time management skills, along with excellent verbal and written communication abilities, are crucial. Experience in wellness coaching, group learning facilitation, and proficiency with computer software are important aspects of this position.
